Enable Health Alliance Inc. provides comprehensive care coordination services to individuals of all ages and abilities. Using our person-centered philosophy and method, we work with individuals and their families to achieve their goals for independence, health, and meaningful community life.
Children & Youth
Enable Health Alliance Inc. partners with families and children and youth with special health care needs to navigate complex systems and connect individuals with the necessary supports and services to thrive at home and in their communities. Our highly-skilled, licensed professionals include Doctors, Social Workers, and registered nurses who understand that each family and child are exceptional. Using a person-centered planning approach, we work with each individual and family to tailor a plan of care centered on attaining personal goals and aspirations. The ways that we support are as follows:
- Assist with coordinating and participate in medical appointments
- Provide community resources Attend and coordinate IEP meetings
- Ensure communication with and among the medical home team is clear, frequent, and timely
- Support, prepare and plan for all transitions, including early intervention to school, hospital to home, pediatric care to adult health care
- Educate children, youth, and families on self-management skills

Adults
Enable Health Alliance Inc. Adult Community Services team supports individuals with disabilities and complex needs to help them achieve optimal quality health, affordable housing, and meaningful community life. We work with adults who live in the community and want to remain independent and others who live in a Maryland nursing facility or are homeless and wish to move to a home. Our highly trained Housing Coordinators, service coordinators, and Health Coaches navigate complex systems and connect individuals with the essential supports and services to thrive at home. With our specialized person-centered planning abilities, we work with each individual to customize a care plan centered on attaining personal goals and aspirations. Our team works together to:
- Identify and secure affordable and accessible housing, provide housing supports, and resources
- Assist with the transition from a nursing facility to a home in the community
- Develop a person-centered plan based on needs, preferences, and goals to help ensure health and safety and facilitate community inclusion
- Assist with obtaining environmental assessments, home-delivered meals, and assistive technology
- Help coordinate medical appointments and assist with obtaining durable medical equipment and medical supplies

SENIORS
Enable Health Alliance Inc. Adult Community Services team supports the seniors in our community to achieve optimal quality health, affordable housing, and meaningful community life. We work with seniors who live in the community and want to continue independent and others who live in a Maryland nursing facility or are homeless and wish to move to a home. Our vastly trained Service Coordinators and registered nurses can navigate complex systems and connect individuals with the necessary support, resources, and services to thrive at home. Using a person-centered planning approach, we work with each individual to modify a care plan centered on achieving personal goals and aspirations. Here are ways we can help:
- Provide transition funds services
- Locate and secure affordable housing or housing supports
- Assistance with coordinating and participating in medical appointments
- Assist with the transition from a nursing home to a home in the community
- Ensure communication with and among the medical home team is clear, frequent, and timely
